In order to submit a request on behalf of your customer, please e-mail the following to cfrb@lgm.ca:

  1. Declaration form (attached)
  2. Customer's vehicle insurance coverage
  3. Customer's original purchase agreement
  4. Specification of the refund option choice (if applicable)
